I made a trip to Italy in 2012, rented a car there, and after the trip got 3 or 4 fine notices in mail, one of them also from Pisa. All for driving in the center of town without the permit. There is an obscure and small sign that they have in Italy, limiting traffic in the city center. Tourists usually don't know about it and get caught.
First, the rental car company charged my card $75 for "providing DL information to government on request". I quickly changed the card number after the first charge to avoid further charges. And fines themselves were in hundreds. I never paid any of them. They kept sending papers many times, over and over again. So I had to return the letters several times with "No such person/Wrong address" written on them to get rid of them.
So far nothing happened. I doubt anything will ever happen. So I would say don't pay.
I also heard bad things about the same rental car company there "Sicily by car". They charge exorbitant fees for all possible reasons (like in my case), and if you are foreign, they would even go as far as sending the forged papers to the credit card company in support of the fraudulent charges. Avoid them if you ever go to Italy.
