Car rental and insurance?

Awhile back someone posted about AMEX's car rental loss and insurance plan for $19.99, wherein AMEX acts as a primary carrier. Anyway, I just checked into it and was told they don't offer liability insurance.

We are going to Aruba. Outside the country our personal car insurance is not valid. What coverage is advisable? of course, I don't intend to have an accident...but just in case, I would want something that covers damage to the other car.

AMEX gold and platinum cards have included 45 days CDW, loss, theft, etc. insurance. Well worth it if you rent a lot - I'm saving over $12 PER DAY in Europe by paying for my car rental with my AMEX platinum card.
