I take care of my two aunt's financial affairs. They moved to Assisted living in January. The place will only order thru Omnicare. My aunt has CVS/Caremark and they will only pay for orders submitted thru them. I finally got a nursing home variance, but there are claims from January I need to refile by hand. Omnicare says they can only refile within 45 days. Caremark promised to mail the forms to me, but didn't. they said get a generic form from the drugstore. Drugstore doesn't know what I am talking about. Nothing online for Caremark or Medco. It is a couple of hundred dollars and I need to get these submitted. Any link to a generic claim form to mail to Caremark?

I don't think it is the correct form.
They did tell me they are not available on line. I don't understand how something so simple can be made so difficult.

Ah, but that's how so much of profit-driven healthcare is run. They don't want to make things easy because, then they might actually have to pay. They don't care about overall costs to the system, just so long as it doesn't come out of their little "silo" as it were

I am going crazy.
I do not see a claim form on the Omnicare site, but I am going to call headquarters.
I went to CVS pharmacy to see if they had a claim form, nope.
The Omnicare center that bills us, never returns a phone call.
Caremark was supposed to mail me the forms, however the order is in to mail to my aunt in Tucson, but in two weeks they haven't mailed it out yet. and they can't put in a new order to this order is out of the system. meanwhile, we are heading to Wisconsin in a week. so i will have to write my aunts (they cannot hear on the phone) to mail the form to me if she ever gets it.
I could fax it, I could attach to an email, but they won't let me do any thing easy. I am beginning to woner if the form even exists.Comment
