  • Trigeminal Neuralgia?

    A few months ago I began to experience shooting pain that felt like a tuning fork on the left side of my face. It happened a few times a day. I went to 2 dentists who took xrays (one took a panorex) and could not find anything abnormal. As time went on, I began to get these pains when I moved my face a certain way, such as when rubbing my cheek when applying makeup, or when blowing my nose and pushing the nose up on the left side. The pains felt like someone was hitting a tuning fork against my teeth. They only lasted a second or two.
    On Tuesday evening I had a horrific episode. I don't remember what triggered it, but it felt like someone put live electric wires in to my face. The searing pain lasted about 10 seconds, followed by intense throbbing for about 2 minutes. The throbbing was centralized over a molar that has a crown on it. After this episode, I was wiped out. On Wed. morning I had the same experience after I got out of the shower and was drying my face with the towel. I went back to the dentist and he banged on my teeth - nothing. He poked and prodded and nothing. But when I began to talk to him and moved my mouth a certain way I had the pain again that almost made me faint. It was good that he was able to see it, but he said he really doesn't think it's a tooth problem. He said there may be an infection low in the sinus that is causing pressure. He gave me Augmentin and told me if the roots were infected the med would take care of that also. After I left his office I had about 12 more episodes, one after the other, with the throbbing lasting for hours. I called him, and he called me in Vicodin which I took but it really doesn't work because the pain is episodic and really severe. He mentioned neuraglia. Yesterday I had 3 episodes of the pain, and I am learning not to touch my face or open wide to eat or talk. Each episode was as severe as the previous. This morning I had an episode in the shower when the water ran across my left cheek! I called my primary who told me to see a neurologist to rule out trigeminal neuraglia. I have an appt. for Tuesday afternoon. In the meantime, I am trying not to talk or move my mouth. I am tempted to insist that the dentist pull the tooth I think is the culprit just in case it is that. He is reluctant because he thinks the tooth may not be the cause. I am wondering if anyone has any experience with anything like this. Any information would be appreciated. I have a high tolerance for pain, and I am at my wit's end here! Thanks in advance - Jacki

    Go see an ENT doctor. There is also a jaw doctor.

      Long time ago

      Many years ago (35 etc) I had a similar problem. Dentist was about to pull tooth, but sent me for x-ray of sinuses. It was a bad sinus infection. Hope you find out cause and it gets taken care of.

        Jackie, you have trigeminal neuroligia.....
        If it flares up again, go to the E.R. - tell them the chain of events and get medication - tegretol or neurontin....Vicodin/Percocet will help for the pain PRN and after 2-3 days the other medication will 'kick in' and you will feel better.
        I found that after I saw the neurologist, he put me on the long acting/time release form of Tegretol and I was able to function normally - I was on the medication 3 months and then tapered off for another month. Once you have had an episode, it is likely that it will happen again.
        I have had it twice - but many years apart.

        Edited to add I also thought it was a dental problem as the symptoms mimicked how I felt when I needed a root canal...went to the dentist and he found nothing wrong..ended up in the ER about 4 days latter because the pain changed from intermittent to almost constant!!!

          I can't believe I have it again!

          I started to have the symptoms - very mild - a few weeks ago.
          But the last 3 nights have been HORRIBLE and after being awake until 3:30 am this morning I finally called my NP.

          When she looked in my records, the last time I had this was 1997!

          Started the medication this evening and hoping for the best.... several people at the MD office suggested acupuncture - has anyone tried this?
          Luckily I know that drugs are my friends in this but it is just annoying to have this again

            I've referred patients to rule out sinus infections.
            Also, have Rx'd Neurontin with success, however, side effects with this med.

              Yes, NP mentioned Neurontin (we often use it in Hospice as an adjunct therapy for neurogenic pain) but since I responded well the last time to tegretol she went with that. Waiting to get a neurologist appt...first opening is Sept 2 so I am on a cancelation list - freakin' crazy! Good thing I am not REALLY sick

                My husband has had TNA for about 8 years now. He too, responds better to Tegretol. It seems to be much worse for him when he goes from one temp extreme to another. His Doc is in Park Slope Brooklyn...let us know if it gets bad and we will get you to him.

                  Pat I am so sorry you are having a flare up. I am pain free since the surgery. They found an artery laying on the nerve. When the artery pulsed, it triggered the nerve to shoot.
                  My neurologist is in Port Jefferson. I can probably get you in soon. Or you can have my appt. that is coming up (next week I think).
                  I am on Neurontin and Cymbalta, and am weaning since the surgery. I've never tried Tegretol, but I did try acupuncture. It didn't work in my case

                    I'm so sorry for your pain. Nothing hurts like a toothache even when it isn't a toothache.

                    I can't imagine getting facial pain from drying your face, or less. Hope you find a solution soon.

                        We know a Neuro Surgeon who uses the Gamma Knife for Trigeminal Neuralgia with success.

                        Hope you can get this resolved soon. We have a niece who had surgery for this (not the gamma knife) and is doing fine so far. That was two years ago, but she still has problems with dizziness and some nerve damage from the surgery. Not perfect, but much better than the horrible pain she had.

                            Side effects??? This med turned me into an idiot, affected my memory and speech, and helped me to gain 30 lbs. However, it controlled my pain for the most part and that to me was worth the nasty side effects. I'm glad to be weaning off it though.
